Action on the first priorities: making calls, gathering information, preparing questions, exploring resources, practicing new skills, connecting with services.

Week 10

Increase participation

Find the ways the person being supported can participate more in their own life, and pick one or more tasks they can begin practicing with the right support.

Becoming, and what's next

Put Week 1 next to Week 12. What changed? What can they do now that you couldn't see before? Who are they becoming, and who are you becoming as their supporter?

You leave with an updated 90-day action plan.
